If you wish to receive a copy of your Notre Dame Academy High School transcript, please complete and submit the form below. A five dollar ($5.00) processing fee is charged per transcript. Payments may be mailed to: 134 Howard Ave., Staten Island, New York 10301, attention: Guidance/Transcript. Once payment is received your transcript request will be processed. Please indicate below if you would like to pick up your transcript or have it emailed to your address.
